The following article involves the life of a bus conductor who met with an tragic accident on his route back to his home and how we POSH hospital helped them in bringing back to his routine.
ON 2ND OF OCTOBER 2021, around 6.50 pm Mr. ILAVARASAN ( 46 yr/ male) who went to his home by two wheeler after completing his shift and collides with an overspped vehicle in the opposite route and severely injured Ilavarasan was first taken to cuddalore GH where initial treatment was given. Then the patient attenders decided to shift to higher centre for better care and management.
IN POSH, the patient came to our hospital by 10.15pm after careful examination by our hospital doctors and staffs, the patient was diagnosed as
1.Crush injury with mangled right forefoot with impaired vascularity and right sided 1,2,3,4, metatarsal fracture with ankle joint avulsion.
2. Right hand 3rd finger near total amputation.
3.Right midshaft of clavicle fracture.
Our emergency team rushed to the hospital and the patient was prepared for surgery around 1.30am. The surgery was planned in two stage process.
1.Wound debridement and k wire fixation of the 1,2,3,4th metatarsal by our plastic surgery team led by Dr.vijayaraghavan and his team
2.ORIF with clavicle plating was done by the orthopaedic team led by Dr. V. Veerappan and his team.
Postoperative period was uneventful and treated was treated with antibiotics and regular wound care.
On post operative day 5, the wound was healthy and the plastic surgery team planned for external fixator of the right ankle and flap cover of the right foot.
The patient was discharged on 18.10.21 with oral medicines and explained about proper wound care and physiotherapy.
The patient once again admitted on 15.11.21 for external fixator removal.
Aggressive physiotherapy was started by our chief physiotherapist Dr. Murugaraj and his team.
The patient was mobilized well and after a period of two months the patient was able to do his day to day activities. Now after four months the patient is able to do well and planning to resuming back to his duty.
As to summarize,

proper triage and emergency care within the first, most critical hour, of a patient’s arrival at the hospital.

Examine, diagnose, treat life-threatening injuries as soon as they are diagnosed.

Use simplest treatment possible to stabilize patient’s condition.

Prepare the patient to operation theatre and shift as early as possible.
